The small thyroid gland located in the neck produces hormones that are critical for growth, metabolism and brain development. In children, an underactive (hypothyroidism) or overactive (hyperthyroidism) thyroid can affect growth, school performance and overall health. Because the symptoms are often insidious, recognizing them is important.

Hypothyroidism (Underactivity)

The symptoms set in slowly:

  • Fatigue, weakness, feeling cold
  • Constipation, dry skin
  • Weight gain, slowed growth (in height)
  • Decline in school performance, reduced attention

Congenital hypothyroidism is caught early through the newborn heel-prick screening; for this reason, newborn screening tests are very valuable.

Hyperthyroidism (Overactivity)

  • Palpitations, sweating, intolerance to heat
  • Weight loss despite a good appetite
  • Restlessness, hand tremor, sleep problems
  • Swelling in the neck (goiter) may accompany it

What Is a Goiter?

A goiter is an enlargement of the thyroid gland; it appears as a swelling noticed in the neck. It can occur when the thyroid is underactive, overactive or working normally; its cause must always be investigated. Any swelling noticed in the neck should be shown to a doctor.

Diagnosis and Follow-Up

The diagnosis is made with a simple blood test (thyroid hormones) and, when needed, a thyroid ultrasound. Most thyroid problems are treatable; with regular follow-up, the child's growth and development are protected. For slowed growth and short stature, a holistic assessment is carried out as part of a developmental evaluation.

What Families Should Do

  • If you notice unexplained fatigue, slowed growth or swelling in the neck, consult a doctor.
  • Do not skip newborn screening tests.
  • After diagnosis, adherence to the medication/follow-up plan determines the outcome.
